We are hiring a Licensed Practical Nurse for a Home Health position providing skilled nursing care to patients in their homes in the New Kensington area. This position is eligible for a $10,000 first year success bonus to reward the accomplishment of key achievements during your first year!
What You’ll Do:
Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you
Perform comprehensive patient assessments including vital signs, medication management, wound care, and overall health status.
Educate and support patients and caregivers on disease management, medication administration, and prescribed treatments to encourage adherence and confidence in care routines
Maintain accurate, timely, and compliant EMR documentation
Work independently, but never alone
Travel to patient homes within assigned territory

LPN Qualifications
Active Licensed Practical Nurse (LPN) license in the state you work
Two years of LPN experience and the clinical expertise to deliver quality patient care
Current driver’s license and ability to spend ~20% of your day driving to/from patient locations
