What you’ll do in this role:
- Meet with patients and families as scheduled by Journey, Palliative, and Hospice
- Conduct patient/caregiver visits to explain and discuss programs
- Implement appropriate patient follow up: phone calls, visits, etc.
- Receive and complete intake information for Hospice referrals
- Function as a knowledgeable resource person and mentor to Home Health, Hospice, and Palliative peers to educate on Residential offerings in relation to advanced chronic illness
- Maintain relationships with all existing referral sources
- Actively prospect for new referral sources based on the Agency’s scope of service
We are looking for a compassionate Hospice and Palliative Liaison with:
- High school diploma/GED
- Prior Home Healthcare/ Hospice or related work experience in the healthcare industry preferred
- Understanding of the Hospice philosophy and or Hospice experience preferred
- Empathetic, non-judgmental, tactful, responsible, and organized
