What you’ll do in this role:
• Actively communicate with referral sources on plan of care for their patients and the care team members
• Update referral sources on any changes to the care team and if the patient has been discharged or expired
• Provide customer service related to CAHPS survey
• Serve as the liaison between company and patients/caregivers
• Communicate Admissions and Discharges per protocol
• Understand and effectively explain all programs offered by the company
• Comply with Company’s Core Values and Core Competencies
We are looking for a compassionate Hospice Customer Service Specialist with:
Associate degree preferred
Minimum of three (3) years customer service experience
Familiarity with medical terminology is preferred
Excellent customer service and verbal communication skills
