It’s Just Better Here. We’re hiring for a Casual Dietician to join our dynamic AHN Healthcare@Home team. This role provides nutritional and dietitian services to patients and caregivers in the home via serving as a resource and/or consultant to the staff or making field visits. Develops nutritional and dietary policies, procedures, and protocols for the organization. In partnership with Allegheny Health Network, AHN Healthcare@Home is an accredited recognized leader in home health and hospice.
Job responsibilities:
To ensure all patients are seen according to plan of care
Review all referrals and document and maintain all client records
Interface with community agencies and compile available resources for patient information.
Complete all documentation for patient charts.
Perform social Nutritional / Dietitian duties as required.
Communicate updates, revisions, and/or changes to Interdisciplinary Team: RN’s, PT’s, OT’s, SLP’s, and the Supervisors within 48 hours of visit.
Develop and implement nutritional policies, procedures, and protocols that adhere to Medicare, regulatory and credentialing standards and criteria.

What we are looking for:
Graduate of an accredited dietitian program and registered to practice within the state
Minimum of two years work-related experience
