Clinical Field Educator

Graham Healthcare Group is hiring a Home Health Clinical Educator, RN to join our dynamic team. The Home Health Clinical Educator, RN will partner with our field staff and leaders to provide the best support to the patients that we serve. This position will be in our Grand Rapids, MI office location.
Home Health Clinical Educator, RN Job Responsibilities:
Trains in assessment, planning, intervention, and evaluation techniques
Communicates with Staff Development Manager & Specialists related to orientee’s completion of the field education guide
Assists Staff Development Specialist in the development of orientee’s schedule to include scheduled visits with other departments in agency
Educates orientee on how to develop and revise the plan of care in response to changes in the individual's status or response to care, and communicate and update physician and other disciplines
Reviews charting expectation and computer documentation with new providers
Instructs orientee in case management practices; organize daily assignments, setting priorities based on individual and overall needs of the clients
Coordinates assistance from preceptors as necessary
Mentors current employees with any identified learning need in day-to-day operations.
Qualification Requirements:
RN License in the state in which you work
Previous Home Health Experience Preferred
Previous Education Experience Preferred
At least 2 years of nursing experience
