Responsibilities

  • Deliver skilled physical therapy and rehabilitation therapy in patients’ homes

  • Develop and implement individualized treatment plans

  • Collaborate with an interdisciplinary care team

  • Document patient care using EMR system (HCHB)

Qualifications

  • Licensed Physical Therapist in Pennsylvania

  • Minimum 1 year of clinical experience

  • Valid driver’s license and reliable transportation
