Responsibilities
Deliver skilled physical therapy and rehabilitation therapy in patients’ homes
Develop and implement individualized treatment plans
Collaborate with an interdisciplinary care team
Document patient care using EMR system (HCHB)
Qualifications
Licensed Physical Therapist in Pennsylvania
Minimum 1 year of clinical experience
Valid driver’s license and reliable transportation
