ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned as necessary.
- Accurately completes required start of care documentation on new admissions, post-hospitalization. Is proficient in HCHB processes.
- Assesses the patient for home tele-monitoring, incorporates tele-health into plan of care, and ensures notification of patient admission to the tele-health program to the tele-health department in established timeframes.
- Develops and implements the patient care plan in cooperation with other Agency professionals and paraprofessionals who will be providing care to the patient. Identifies additional disciplines/care programs for the patient and implements such using such guidelines.
- Observes signs and symptoms and reports, to the physician, reactions to treatments including drugs and changes in the patient's physical and emotional condition. Obtains, as needed, physician orders to adjust the plan of care. Communicates changes to the plan of care to the nursing supervisor, case manager and other members of the home care team or program representatives. Medication reconciliation and follow up if discrepancies.
- Teaches, supervises, and counsels the patient and family members regarding the nursing care needs and initiates preventive and rehabilitative nursing procedures as appropriate for the patient's care and safety.
- Assesses and assigns Home Health Aides as appropriate by providing a written plan of care and specific duties the Aide is to perform for each individual patient.
- Initiates preventative and rehabilitative nursing procedures, as appropriate, for the patient’s care and safety, according to the Agency’s policies and procedures.
- Communicates with supervisor and, as indicated to other members of the home care team, concerning problems of the patient(s) on a timely basis and communicates information using current process and technology available to the organization.
- Attends and participates in office staff meetings, patient care conferences, record audits, utilization review, performance improvement, in services, workshops, and seminars.
- Satisfies CEU requirements of the State Board of Nursing.
- Keeps up-to-date on reimbursement criteria and charting needs for all patients.
- Strives to improve nursing care through continuing education, attendance at workshops and conferences, active participation in professional and related organizations, and individual research and readings.
- Adheres to the Agency’s personnel policies and Compliance Plan.
- Displays a high degree of courtesy, tact, and knowledge of services provided by the agency in all contact with staff, patients, and visitors.
- Respects patient rights, and property.
- Must call in report daily to Supervisor or scheduler.
- Contacts the Nurse Supervisor to inform him/her of alternate schedule for patient visits during vacation, sick days, or personal days. Immediately reports any schedule change or inability to complete assignment as scheduled.
- Accepts changes/additions to schedule during staffing difficulties when necessary.
- Organizes daily visit schedule to minimize travel time and mileage. Maintains productivity according to work status. You will be expected to travel where business needs are located in order to reach your weekly productivity.
- Must meet weekly productivity points per week. Equivalent of three (3) SOCs or ROCs per day.
- Must work weekend/holiday as required. One weekend per month, three (3) SOCs or ROCs on Saturday = One (1) day off preceding week and three (3) SOCs or ROCs on Sunday = One (1) day off following week.
- Untimely or poor documentation is subject to disciplinary action, up to and including termination.
- Must travel to where business needs are located in order to reach productivity. RHH will always do their best to keep clinicians within a reasonable territory.
- Performs other duties as assigned by supervisor.
QUALIFICATION REQUIREMENTS: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. - Must be able to meet the physical demands of providing nursing care, ambulating patients, transferring patients, assisting with exercises, etc.
- Must be able to transfer patients as needed according to proper techniques of body mechanics to promote patient and staff safety.
- Must be able to use good and sound clinical judgment.
- Must be able to document accurately and appropriately.
- Must follow up as needed and appropriately with doctor’s office, clinical and management staff pertaining to any concerns regarding patient care.
- Must hold, in good standing, a license as a Registered Nurse, issued by the state in which you work
- CPR certified.
- Must be empathetic, non-judgmental, tactful, responsible, and organized.
- Must understand the team approach and be a supportive team member.
EDUCATION and/or EXPERIENCE:
- Must possess an Associate Degree from an accredited School of Nursing.
- A minimum of two (2) years of experience working as a Registered Nurse including some medical/surgical experience and/or an acute care or home care setting is preferred.
CERTIFICATES, LICENSES, REGISTRATIONS:
Must have and maintain a valid Driver's License, maintain automobile insurance coverage and have access to a reliable automobile. Must have and maintain in good standing professional license, certificate or registration.

While performing the duties of this position, the employee travels by automobile and is exposed to changing weather conditions. Employee will be required to drive daily for meetings and/or visits to residential homes. Employee will work in varied residential environments based on assigned caseload.
