What you’ll do in this role:
Meet with patients and families as scheduled by Journey, Palliative, and Hospice
Conduct patient/caregiver visits to explain and discuss programs
Implement appropriate patient follow up: phone calls, visits, etc.
Receive and complete intake information for Hospice referrals
Function as a knowledgeable resource person and mentor to Home Health, Hospice, and Palliative peers to educate on Residential offerings in relation to advanced chronic illness
Maintain relationships with all existing referral sources
Actively prospect for new referral sources based on the Agency’s scope of service
We are looking for a compassionate Hospice and Palliative Liaison with:
High school diploma/GED
Prior Home Healthcare/ Hospice or related work experience in the healthcare industry preferred
Understanding of the Hospice philosophy and or Hospice experience preferred
Empathetic, non-judgmental, tactful, responsible, and organized
