We are hiring an Admission Registered Nurse (RN) for a Hospice position providing end-of-life care to patients in their homes in the Troy, MI area. This position is eligible for a $9,000 first year success bonus to reward the accomplishment of key achievements during your first year!
The schedule for this position is Monday through Friday, 4pm - 12am.
What You’ll Do:
- Deliver compassionate, patient-centered care in a hospice setting, collaborating with an interdisciplinary team dedicated to comfort, dignity, and quality of life
- Conduct comprehensive hospice admission assessments, including clinical evaluations, medication reconciliation, and development of individualized plans of care aligned with physician orders
- Educate and guide patients and families through the hospice admission process, setting expectations, explaining the hospice philosophy, and ensuring they feel informed, supported, and comfortable with the transition to end-of-life care
- Apply strong clinical judgment and compassionate critical thinking to facilitate timely admissions, address immediate needs, and ensure accurate, thorough, and compliant EMR documentation
- Initiate evaluations to other disciplines when indicated
- Work independently, but never alone
- Travel to patient homes within assigned territory

Admission RN Qualifications
- Active Registered Nurse, RN license in the state you work
- Two years of RN experience and the clinical expertise to deliver quality patient care, hospice experience preferred
- Current driver’s license and ability to spend ~20% of your day driving to/from patient locations

The low end of the range includes compensation related to salary and first year success bonus. The high end of the range is inclusive of productivity bonuses and additional shift work that is paid at a premium (i.e., after hours, weekends, on-call). *Compensation potential varies by market.
