What you’ll do in this role:
- Serves as the first line of customer service for external and internal customers.
- Professionally responds through electronic referral platforms within the specified time requirements.
- Performs reverse marketing in coordination with the local BD Leader to reinforce messaging, stay top of mind and present information where it may be of value.
- Meets assigned key performance indicators (KPIs).
- Receives and documents patient referrals to hospice services through electronic referral systems, ensuring compliance with applicable coverage criteria.
- Coordinates and communicates with Hospice Centralized Intake Coordinators or designee to enter referrals into Homecare Homebase.
- Gathers appropriate information to assist the clinical team in scheduling an evaluation visit.
- Provides the appropriate information from the eReferral system to the Hospice Centralized Intake Coordinator to ensure complete referral information.
We are looking for compassionate Hospice Intake Analyst with:
- High School diploma or GED equivalent.
- Experience working in an environment where timeliness and quality of response were measured and impacted business results.
- Hospice or Home Health experience, preferred
- Experience customer facing in sales or operations, preferred
- Reliable internet service and ability to office from home with minimal disruption to the workday.
