SUMMARY
The Regional Intake Manager is responsible for the team assigned to prepare, transcribe, and update patient referral information for the new pending patient or to facilitate timely resumption of home health services. The Regional Intake Manager under the direction of the Vice President of Intake, is to support both professional and administrative support personnel by interviewing applicants and recommending hires, provides training, make work shift assignments, approves absences, and evaluate the performance of the team.
REGIONAL INTAKE MANAGER RESPONSIBILITIES
Lead and Manage Teams
Leads and supervises intake staff by providing/coordinating training, performance evaluations, time and attendance, and supportive coaching.
Manages and maintains staff schedules and time off
Train new and current Intake Employees on new and current processes using the most up to date job aids and SOPs
Ensure staff have access provision and equipment to perform their roles
May be required to participate in rotational holidays and weekends, if needed
Coordinates and Communicates with Key Stakeholders
Maintain clear and effective communication with intake coordinators, patients, families, sales, liaisons, insurance, and scheduling
Collaborates with departments to ensure timely starts and resumptions of care
Data Management
Ensure accurate, timely, and complete data entry and management of client information for intake workflow including no-admissions, and delays
Monitors and manages daily workflow using reports and dashboards to ensure patients are scheduled on time
Problem Solving
Address and resolve issues or concerns that arise during the intake process including obtaining or clarifying orders for home health services and treatments
Identifies home health clinical requirements on the patient referral to support care transition visit needs
Appropriately applies critical and clinical judgment to resolve day-to-day concerns
Demonstrates appropriate steps to escalate concerns to the designated department leader
Identifies clinical grouping or clinical focus of care for the anticipated home health plan of care prior to start or resumption of care services using appropriate supportive medical provider documentation if needed.
REGIONAL INTAKE MANAGER REQUIREMENTS

EDUCATION and/or EXPERIENCE:
Graduate of an accredited nursing program
Active Registered Nurse state license
Minimum of (1) year of clinical experience in home health.
Minimum of (1) year of direct patient/family experience to address customer service concerns
