What you’ll do in this role:

  • Gather and verify accurate patient demographic information, medical history, primary diagnosis, and primary care provider details.
  • Introduce Residential Home Health services to patients and caregivers, clearly outlining the scope of skilled services and coordinating the Start of Care visit.
  • Facilitate additional support services as needed, including ordering durable medical equipment (DME), infusion therapy, and wound care supplies.
  • Collaborate with the sales team to deliver community education to physician offices, assisted living facilities, and other care settings—covering topics such as transitional services, educational in-services, hospice care, and other Residential programs.
  • Provide bedside patient education, arrange follow-up appointments with primary care physicians, and ensure seamless communication during care transitions.
  • Assess the suitability of skilled home care and hospice services, and collaborate with the clinical team on the management of complex, chronically ill patients to reduce hospital readmissions.
  • Lead and coordinate team events, mentor and coach the Transitions Nurse Liaison (TNL) team to enhance quality and streamline patient transitions; assist in adopting new technologies to optimize workflow and ensure exceptional care continuity.

We are looking for compassionate Transitional Nurse Liaison Manager with:

  • Associates Degree required, Bachelors’ Degree preferred.
  • RN license required
  • Three (3) years of experience in home health preferred.
  • Minimum of one (1) year of experience in an acute care setting; 3 years of recent experience in an adult critical care or medical-surgical setting preferred.
  • Minimum of one (1) year of experience in a customer service capacity; 3 years of experience as an RN Liaison preferred.
